Many organizations fear obvious AI errors: answers that are clearly nonsensical, texts full of contradictions, or blatant factual mistakes. These are usually caught and corrected quickly. The truly dangerous risk lies in the plausible errors that nobody questions.

An AI-generated risk report looks professional. An automatically produced contract analysis sounds competent. Incorrectly transcribed statements in meeting minutes are treated as fact. In all of these cases, the error is not obvious - it is embedded in an output that inspires trust. That is precisely why it goes unnoticed. And that is precisely why it is so dangerous.

In a business context, this phenomenon quickly becomes a governance problem: flawed analyses feed into decisions, incorrectly summarized contracts get signed, risky gaps in documentation go undetected. Not because the system is broken - but because it optimizes for probability rather than truth.

Four Governance Measures for AI in Regular Operations

Organizations that want to deploy AI systems responsibly must actively manage the risk of plausible errors. This does not mean rejecting AI tools entirely, but rather establishing structured frameworks.

First: Define clear use boundaries. AI systems should only be used in precisely scoped applications where their strengths and weaknesses are understood. Where precision is critical - such as in legal or regulatory documents - AI should serve only as a first draft, never as the final source.

Second: Establish review checkpoints for critical outputs. Every AI-generated output that feeds into decisions needs a human review step. This applies especially to meeting transcripts, automatically generated reports, and AI-assisted risk analyses.

Third: Recognizable uncertainty instead of blind trust. Good AI governance means deploying systems in ways that keep uncertainties visible. Users must know when an AI output is uncertain and how to recognize it.

Fourth: Clear accountability chains for edge cases. When an AI output leads to a wrong decision, it must be clear who bears responsibility. This requires documented processes, defined roles, and a culture that actively encourages critical questioning.
